To register and use the password-protected services offered on this website, we collect the following information from you: First and last name, Insurance Member identification number, date of birth, company or employer name, and e-mail address. For certain AultCare plan offerings, including Marketplace plans, we offer online enrollment tools to facilitate the application process. These online tools are voluntary. If all of the requested information is not provided through the online enrollment tools, alternate methods of information collection may be used to compete the enrollment process.
